Raydient LLC DBA Raydient Places and Properties 1 Rayonier Way Yulee, FL 32097-0002 and Wildlight 1 Rayonier Way Wildlight, FL 32097-0002 permit #139976-84. This public notice is to solicit Pre-Qualification applications from Bidders as per the State

Requirements for Educational Facilities (SREF, 2014), Chapter 4.1 (1)(a) and Nassau

County School District Administrative Rule 6.08 as follows.

A.      Prequalification of Contractors. The Board shall pre-qualify contractors on an annual basis or for a specific project. This section is applicable to bids, construction management, design build, and any other construction services application.

B.        Selection Process. Those contractors desiring to bid on Board projects must be prequalified. The Board may reject any application that contains inaccurate information.

C.      Application. Each contractor, firm or person requesting prequalification shall submit an application packet. The application shall include the following:

1.     Detailed information setting forth the applicants competence, past performance, experience, financial resources, and capability, including a Public Entity Crime statement and references.

2.       Audited financial information current within the past 12 months, such as a balance sheet and statement of operations, and bonding capacity. The requirement for financial information may be satisfied by the contractor providing written verification of the contractors bonding capacity.

3.       General information about the contractor company, its principals, and its history, including state and date of incorporation, regardless of whether the contractor is a resident or non-resident of the geographic area served by the Board.

4.       Every contractor and subcontractor desiring to enter into a contract with the school district shall register with and use the E-Verify system to verify the work authorization status of all newly hired employees. Contractors entering contracts with a subcontractor must have an affidavit from the subcontractor stating the subcontractor does not employ, contract with, or subcontract with an unauthorized alien.

a.       Contractor must provide evidence of compliance with Florida Statute Section 448.095. Evidence may consist of, but is not limited to, providing notice of Contractors E-Verify number.

b.      Contractors entering into contracts with a subcontractor must have an affidavit from the subcontractor stating the subcontractor does not employ, contract with, or subcontract with an unauthorized alien.

c.       Contractor shall provide a copy of sub-contractor affidavit to the School Board upon receipt and shall maintain a copy for the duration of the Agreement.

d.    Failure to comply with this provision is a material breach of an Agreement, and School Board may choose to terminate the Agreement at its sole discretion. The contractor may be liable for all costs associated with the School Board securing the same services, inclusive, but not limited to, higher costs for the same services and rebidding costs (if necessary).

5.       Contractor trade categories and information regarding the state and local licenses and license numbers held by the applicant.

6.        A list of projects completed within the last five (5) years, including dates, client approximate dollar value, size and reference name for each project.

7.        Certificates of insurance confirming current workers compensation, public liability and property damage insurance as required by law.

8.        A list of all pending litigation and all litigation within the past five (5) years, including an explanation of each. Litigation initiated by the contractor to protect the contractors legal rights shall not be used as a basis for rejecting prequalification.

9.      The completed application and financial information shall be attested to and signed by an authorized officer of the company, the owner, or sole proprietor, as appropriate, and the signature shall be notarized.

The School Board of Nassau County is calling for proposals for PRE-QUALIFYING OF GENERAL CONTRACTORS for the purpose of bidding on construction projects in Nassau County Florida.  This will be a single year pre-qualification to the approved Contractor(s).

PROJECTS:  The Contractor(s) selected for this annual pre-qualification will be eligible to bid on any assigned projects, during the one-year period, having estimated costs not exceeding the threshold amounts of $2,000,000.00 (Construction) provided for in §255.32 Florida Statues.

This permit authorizes the permittee to operate a 26.31 MGD maximum daily flow of non-process wastewater, process wastewater, and stormwater to Amelia River. The facility is located at latitude 30o 39' 45.12" N, longitude 81o 28' 12.23" W at 10 Gum Street, Fernandina Beach, Florida 32034-4285 in Nassau County. The Department has assigned permit file number FL0000701-012-IW1S to the proposed project.

(e) A concise statement of the ultimate facts alleged, including the specific facts that the petitioner contends warrant reversal or modification of the agency's proposed action;

(f) A statement of the specific rules or statutes the petitioner contends require reversal or modification of the agency's proposed action, including an explanation of how the alleged facts relate to the specific rules or statutes; and

(g) A statement of the relief sought by the petitioner, stating precisely the action that the petitioner wishes the agency to take with respect to the agency's proposed action.

The petition must be filed (received by the Clerk) in the Office of General Counsel of the Department at 3900 Commonwealth Boulevard, Mail Station 35, Tallahassee, Florida 32399-3000, or via electronic correspondence at Agency_Clerk@FloridaDEP.gov. Also, a copy of the petition shall be mailed to the applicant at the address indicated above at the time of filing.

Time Period for Filing a Petition

Petitions filed by any persons other than the applicant, and other than those entitled to written notice under Section 120.60(3), F.S., must be filed within 14 days of publication of the notice or within 14 days of receipt of the written notice, whichever occurs first. The failure to file a petition within the appropriate time period shall constitute a waiver of that person's right to request an administrative determination (hearing) under Sections 120.569 and 120.57, F.S., or to intervene in this proceeding and participate as a party to it. Any subsequent intervention (in a proceeding initiated by another party) will be only at the discretion of the presiding officer upon the filing of a motion in compliance with Rule 28-106.205, F.A.C.

Extension of Time

Under Rule 62-110.106(4), F.A.C., a person whose substantial interests are affected by the Department's action may also request an extension of time to file a petition for an administrative hearing. The Department may, for good cause shown, grant the request for an extension of time. Requests for extension of time must be filed with the Office of General Counsel of the Department at 3900 Commonwealth Boulevard, Mail Station 35, Tallahassee, Florida 32399-3000, or via electronic correspondence at Agency_Clerk@FloridaDEP.gov, before the deadline for filing a petition for an administrative hearing. A timely request for extension of time shall toll the running of the time period for filing a petition until the request is acted upon.
